The cylinder seal provides a much more effective and comfortable experience. It is no problem moving the cylinder left and right or any other movement if you are shifting position etc. Without this cylinder seal you have to keep the cylinder rim flat against your "equipment/bone" perimeter or you will lose the seal. It works and is very comfortable. It feels delicate as mentioned in other reviews, and so I have used care in handling and storing so as not to tear it.
