What are Cock and Ball Toys?
Cock and ball toys are any devices or toys that are used on the penis or balls. These include cock rings, chastity devices, penis jewelry, ball stretchers, masturbation toys, penis extenders or hallow sleeves and penis enhancement devices such as penis pumps. Some people use these toys as a means to improve or enhance their sexual organs by stretching or pumping them to make them larger. Some people want to improve their sex life and use these toys to help with ED (erectile dysfunction) or PE (premature ejaculation). Others like the S&M (sadism and masochism) aspect of BDSM and opt to use these toys to inflict or experience pain, specifically CBT (cock and ball torture).
First, for almost any toy, you’ll need lube! Lube makes using toys fun and easy and it can be very arousing to get super wet! Lubricants are one of the most underrated sex and intimate play accessories. People often call it oil, liquid, gel or jelly, but the most common name is lube. It’s true that men can produce their own lubricants with spit or seminal fluid. However, the amount of lubricant that spit or seminal fluid provides is limited and isn’t going to be as slippery, long-lasting, or sensational as the products available today. Additionally, there are lubes that are super slippery or have ingredients that cause the cock and balls to feel heated or tingle. When choosing a lube, make sure it is compatible with the toy and never use silicone lube with a silicone toy. To learn more about lubricants, read our Guide to Lubricants!
Cock rings are the next most popular cock toy to be added to someone’s bag of toys. Cock rings are constrictive devices that limit the backflow of blood in the penis in order to help maintain an erection and last longer during sex. They have often been called constrictive rings, adjustable rings, c-rings, couple’s rings, erection rings, penis rings, shaft rings or tension rings. For our purposes, we will just call them cock rings. For those who’ve desired to extend their lovemaking longer and avoid premature ejaculation, cock rings are a simple, cost-effective and easy choice. Not only do they help people who have ED (erectile dysfunction) and those who are highly sensitive or may struggle with PE (premature ejaculation), they also have several other benefits that individuals and couples can enjoy during partnered sex or masturbation.
Penis jewelry, a close relative to cock rings, is worn on the shaft, usually towards the head of the penis. Pressure on the frenulum feels very nice and it causes the glans to swell up. Men can wear penis jewelry for aesthetic or functional reasons. Besides being pleasurable and making their cock look amazing, they can also inflict pain by making the penis bend in an uncomfortable manner and binding it, similar to a chastity cage. They can also include adaptations that cross over into medical fetish by adding a urethral sound that inserts at the tip of the penis and into the urethra; the end of the sound can be seen at the head of the cock and is decorative. Read all about these and more in our Guide to Cock Rings!
Another popular item to wear around the cock are chastity devices or cock cages. People enjoy chastity devices for a number of reasons. The most popular reason is to have or give up a sense of control. Some use it only during bdsm sessions and others do this for weeks or months and consider it part of their lifestyle. It’s usually something that is practiced between two people (the one wearing the cage, usually the submissive partner, and the one holding the key to unlock the cage, usually the Dominant). However, some men will practice this alone and give themselves challenges to see how long they can keep it on.
It may surprise some that the practice of chastity is actually very popular. There are many men that wear cages all day and the people around them never know, and that’s part of the excitement! Wearing a cock cage limits access to the cock and becomes a form of tease and denial. When the wearer of it has it on long enough, they begin to think of the person who owns the key more and more, which can spice up the relationship between them. At some point, they feel a growing sense of desperation and longing and relish the thought of having it taken off. Others pride themselves in how long they can keep it on and start to focus on other erogenous zones, such as their anus. There are many reasons people enjoy chastity. Read more about it in our Guide to Chastity Devices!
Masturbation Toys are any type of device or toy that assists in masturbation or jerking off. The simplest form of masturbation toy is a silicone sleeve that is slipped over the shaft and used to enhance stroking during masturbation. Over time, male masturbation toys have developed in variety and even become high tech! There are masturbation sleeves that have unique and stimulating textures and ridges to give the user a unique experience of pleasure. There are also high tech toys that suction, vibrate, massage, spin and thrust while the cock is inserted. These simulate fellatio, intercourse or offer a new experience that is out of this world! Penis enhancement refers to anything that enhances the girth, length or function of the cock. There are a few devices that will help engorge the cock with blood and help maintain erection (penis pumps and cock rings), there are toys that are worn on the cock to add girth, texture or length without modifying the body (penis sleeves), there are devices that lengthen and straighten the penis through traction (penis stretchers), and there are creams, lubricants and sprays that have numbing agents in them so the penis isn’t oversensitized (this is great for men who suffer from PE).
Penis enhancement toys include penis extenders and penis pumps which are used to make the penis larger, longer or girthier. The penis pumps can be used with a cock ring right before sex to engorge the penis with blood and help maintain an erection. Penis extenders or penis stretchers are devices that stretch and lengthen the penis over time. Wearers of these devices cannot use them during sex and they are primarily a tool used to obtain a specific aesthetic or correct certain medical issues. Penis enhancement devices also include hollow sleeves that can be worn on an erect penis to add girth, length or texture without any physical augmentation of the cock. These can be very fun for those who enjoy fantasy role-play as the man can change his cock to be any style his partner desires! For those who have trouble maintaining an erection, a hollow strap-on is great because it will stay in place even when the erection is temporarily lost. A lot of men enjoy ball stretchers. Stretching your balls has a lot of benefits but it’s also done for aesthetic reasons, too. Many men enjoy having low hanging balls. For some men, their balls are always snug up against their body. When it’s warm, testicles descend (drop). When it gets cold, they ascend (lift). However, for men who have balls that are already up high against their body, the cold can make it feel like their balls are retreating into their body (which is fairly uncomfortable). Other men have large testes, but the skin is too tight and doesn’t allow the testes to hang freely. Of course, other men simply love the look of lower hanging testicles and believe it emphasizes their genitals.
Stretching balls creates a pleasurable, tingling sensation in the testicles and the spermatic cord. It makes the balls more sensitive to touch and and fondling of the scrotum becomes more erotic. After stretching the scrotum and spermatic cord, sperm has a longer distance to travel during ejaculation which extends orgasm can cause a pleasurable, tingling sensation. The concept of ball stretching is similar to how men stretch or lengthen the penis using a penis stretcher. When tension is applied on skin consistently over time, the skin stretches. As the skin stretches, there are micro-tears in the skin and new cells grow as it heals. As the new cells fill in the space that has been stretched, it can make the skin longer and even thicker over time. For those curious about penis stretching, check out our Guide to Penis Enhancement.
Ball stretchers include more than just weights that hold the testicles low and stretch the scrotal sac. They also include CBT (cock and ball torture) devices. While many men enjoy the pleasurable sensation of ball stretching, others truly enjoy the pain of CBT which specifically refers to any pain inflicted on the penis, scrotum or testicles.
CBT includes squeezing, kicking, kneeing, flogging, whipping, crushing, stomping on the balls and cock (especially with boots or heels on), constricting, ball-busting, urethral play, tickle torture, electrostimulation, wax play and even piercing. We won’t go into detail about all of these methods here, but we will talk about crushing, pressing, constricting and stretching using cock and ball toys.
Generally speaking, CBT is an advanced form of erotic play between a Dominant female (a Dommee or FemDom) and a submissive male. For male-male relationships, there is a Dominant male (Dom) and a submissive male, but the latter dynamic is more common. The core desire is for the submissive to give up control and experience pain. Because of the high-risk nature of CBT, it should not be practiced by novices. Ball busting is also called tamakeri and has a huge community in Japan; women will use martial arts kicks to kick the balls until they swell up. Ball crushing has a more medieval feel to it and uses plates and screws to tighten down on the balls and slowly crush them between the metal plates.
Many men around the world fantasize about CBT but are concerned about bringing it up to their partners. Often they will seek out services from a professional Dominatrix or Dom to explore these fantasies more. One benefit of seeking out professional services is that they have the knowledge and experience to help individuals discover what they like and they can do it in a safe way. However, whenever seeking out the services of a professional, ask for references, pay attention to local laws concerning sex work and stay safe (both for your sake and theirs).
However, there are some safe intro-level devices that have less chance of inflicting damage and can be more easily explored with less experienced partners. Using a humbler, for example, is one way of locking the balls in place and limiting the submissive’s movements without too high of a risk for injury. As long as the Dominant is trustworthy and relatively experienced, it should be safe. Ball crusher board is also a relatively safe tool as long as the Dominant listens to the submissive and doesn’t actually crush their balls, but only applies enough pressure to cause discomfort. A parachute is a conical type of ball stretcher that the Dominant can attach heavy weights to; it provides a constant drag and the quicker the weights are added, the more pain is inflicted. For those who practice ball stretching over the long-term, their scrotum is prepared for the stretch. However, for those who aren’t already stretched out, the sudden intense pull on their scrotum and balls can be painful, which is perfectly fine if that’s what they have previously consented to. Another easy and fun way to add some pain to the cock or balls is to use plastic clothes-pins and place them on the scrotal sac or skin of the penis (do not place them on the testicles themselves). They work great when paired with a ball stretcher or cock ring.
Any of the above devices can be combined to create infinite possibilities for ways of playing with the cock and balls and to cause pleasure, pain, or both!
Why You Need Cock and Ball Toys
It can get monotonous to do the same thing all the time. Cock and ball toys allow men to explore a wide range of stimulation and sexual play! There are many ways to play with male genitalia. Some men want sexual pleasure and satisfaction, others want a little bit of pain or restraint, while others want to work on their stamina or sexual prowess. Below are a few fairly common reasons men explore with cock and ball toys.
- Aesthetically Pleasing Look Some men who use ball stretchers find the look of big balls or low-hanging balls attractive. Stretching them with ball stretchers gives them confidence and can feel pleasurable. Some men wear cock rings or penis jewelry because it enhances the look of their penis and makes it look large, engorged and swollen.
- Longer Erections Cock rings limit the backflow of blood which helps maintain an erection longer. Ball stretching also supports longer erections and extended orgasms.
- Premature Ejaculation Stamina training devices like the Fleshlight and other realistic sex toys help train men to know when to slow down or hold back so they don’t blow their load too soon during sex.
- Erectile Dysfunction Cock rings and penis pumps are well known to be aids for those with ED (erectile dysfunction).
- High Tech Masturbation Try something new and explore the most fantastic ways to jerk off! These devices can enhance sex for one person or for couples. Because some of these devices simulate fellatio or intercourse, it can be used with asexual partners, partners who have vaginosis and find sex painful, partners with physical disabilities who want the freedom to have devices that they don’t struggle to position themselves on top of, and of course they can also be used simply for fun!
- Increase Hormonal Secretion of Testosterone Ball stretching has been shown to increase testosterone, increase testicle size and “improve circulation in the inguinal region”. Read more about it here.
- Sexual Arousal Many different cock and ball toys are stimulating and arousing. They either look or feel aesthetically pleasing and give confidence to the individual using them, or they stimulate the genitals and turn them on. Every person has their personal preference, so it’s best to explore options until you find what works for you!
- Enhance Sexual Pleasure After removing a ball stretcher, the low-hanging testes enhance pleasure during sex and give the man’s partner more to feel as the balls slap against them with each thrust. Penis jewelry when worn on the frenulum can also feel extremely pleasurable. Finally, the suction from a penis pump brings blood to the cock and causes it to swell; the sensation of suction can be extremely pleasant in and of itself.
- Prolonged Orgasms Stretching the balls can cause prolonged orgasms. The orgasm itself lasts as long as semen is traveling through the testicles and penis. If the spermatic cord (vas deferens) is elongated via stretching, the sperm has a farther way to travel and the orgasms are prolonged. Additionally, when the scrotum is inhibited from contracting up towards the pubic area, most men feel a pleasant, tingling sensation.
- CBT (Cock and Ball Torture) Ball stretching in particular can be used gently as a way to enhance pleasure, but it can be used in a more extreme way to induce pain in the individual whose balls are being squished, stretched away from the body or put under intense pressure or discomfort in any way. There are also ball crushers that will put pressure on the testicles themselves and “crush” press down on the nuts. This type of BDSM play is not for beginners and not intended to cause long term damage if done by a skilled Dominant.
- Control in a D/s (Dominant/submissive) Relationship Chastity devices or cock cages cause one partner to be completely under the control of lock and key of another partner or Dominant. This type of relationship is highly arousing and many men secretly practice chastity for months at a time.

PVC (polyvinyl chloride) or TPE (thermoplastic rubber aka “skin-safe rubber”): PVC is an easily moldable plastic and TPE is an easily moldable rubber.
To Clean: Clean with mild soap and warm water using a washcloth. Dry after each use before storing.
Safety: This material is porous. PVC is a plastic and TPE is a rubber, meaning they are more porous and sometimes contain chemicals harmful for the body, so make sure you buy ones that are labeled non-porous and phthalate-free or use a condom with it as a barrier. This type of material doesn’t stand up well to extreme heat, so store it in a cool, dark place.
To Clean: First, double check to make sure your vibrating toy is waterproof before cleaning, but most vibrating toys can be cleaned under running water with a mild soap.
If it has batteries: Do not submerge in water or put in the dishwasher. Take out batteries. Hand wash and wipe down with a cloth using soap and water.
If it’s rechargeable: Do not submerge in water or put in the dishwasher (even if it’s water-resistant). Hand wash and wipe down with a cloth using soap and water.
Safety: Pay attention to the material used and use the compatible lube. Use condoms when sharing with a partner or playing with anal.
Leather
- Sexy and a classic
- Durable
- Can stretch and soften after multiple uses for a custom fit
- Porous; harder to keep clean
- Needs to be washed/cleaned after every exposure with bodily fluids.
To Clean: Wipe clean with warm water and mild soap. Let it air dry. Moisturize with saddle soap and mink oil.
PU (polyurethane) Leather & Leatherette
- Vegan, imitation leather
- Less porous than leather; easier to clean
To Clean:
- PU leather can be washed and dry-cleaned.
- Leatherette and PU leather can be wiped down with mild soap and water, then air dried.
Metal (Chrome, Steel, Aluminum)
- Non-Porous, easy to clean, compatible with all lube types
- Temperature sensitive
- Non-adjustable; can be difficult to get off an erect penis
To Clean: Soak or boil in a solution of 10% bleach to 90% water for 10 minutes, place in the dishwasher, or use warm water and soap.

Safety
CBT is not about causing permanent damage!
This type of erotic sado-masochistic play is about inflicting pain, not injury. Make sure everyone involved has had clear communication about what will happen, that both parties know the safeword and are comfortable using it in a scene and that the Dominant is trained and knowledgeable about the type of CBT they are inflicting on the submissive.
Clean all the toys before and after.
Playing with the cock and balls exposes the toys to mucous membranes and bodily fluids. This increases the risk of STI (sexually transmitted infection aka STD) transmission.
Go slow when trying out any new device.
Slowly apply pressure if it is a CBT or ball stretching device. Don’t rush anything. The testicles are very sensitive, so if you go slow you can easily find the edge of pain and pleasure and know when you’ve reached your threshold.
Don’t use rubber bands on your balls.
Don’t use any DIY ball stretching devices, but especially avoid rubber bands as they can constrict blood flow to the point of cutting it off.
If it hurts, stop.
This goes for all toys and devices. Pain and discomfort is an indicator that the area needs attention and something needs to be adjusted or discontinued.
When wearing penis jewelry, pay attention to pain or discomfort.
Similar to wearing a cock ring or any device that restricts blood flow, it is important to take note of discoloration or discomfort. Remove jewelry if uncomfortable and consider getting a larger size.

Expert Tips & Advice
How to put a two-piece metal ball stretcher on:
- If you have a two-part stretcher, have the device open and the screw ready.
- Have a lubricant of choice nearby.
- Take a hot shower/bath; this will cause the testicles to descend away from the body and stretch naturally on their own.
- Find a relaxed position that gives you easy-access to reach your testicles.
- Apply a small amount of lubricant to the testicles, but not too much that it is slippery. Wipe the lube off your hands so your fingers aren’t slippery.
- Make an “okay” sign with your fingers by touching the index finger and thumb right above the testes. Start to gently pull downward and side-to-side so the balls begin to stretch away from the pelvis.
- Hold your testicles with one hand and begin to put the stretcher on.
- If you have a two-piece ball stretcher, grab the half that doesn’t have a hole in it and place it behind the balls near where you’re grasping the scrotum.
- It should sit nicely above the testicles and below the perineum.
- Take the other half of the ball stretcher, line it up with the one in the back to connect them. Make sure both pieces are above the testicles, not around them.
- You may need to pull the testicles down a little to make sure the device is high enough above the balls.
- Once the two halves are together, take your dominant hand and screw it shut.
- If there is still some skin between the two pieces, screw it slightly closed, adjust so the skin is pulled out of the way, then continue to close the gap between the pieces until the device is completely on.
If you have a metal or rubber ball stretcher ring:
- Take a warm shower to help increase circulation and encourage the testicles to descend.
- Apply toy-compatible lubricant to the testicles.
- Gently pinch the bottom of the scrotum and pull it through the ring.
- Then, pull one testicle at a time through the ring.
- Adjust the skin until it is comfortable.
It is common to pinch the skin of the scrotum while putting on ball stretchers. Especially for new users, it is common for skin to get pinched. Going slowly and following the directions above does help. Keep gently pulling the scrotum down and away from the shaft of the penis as you slowly close the stretcher around the skin.
Use a mirror while putting the ball stretcher on. This will help you see what you’re doing and adjust where necessary so you don’t pinch your skin.
Make sure the ball stretcher fits correctly. You want it to be tight enough to feel the pressure and prevent slipping off, but not loose enough that it doesn’t cause pain or discomfort.
Don’t screw the stretcher too tight. Make sure you aren’t screwing it in so tightly that it is difficult to get off when you want/need to remove it.
Only wear the weighted stretcher for about 10-20 minutes the first few times. It’s best to give yourself a time limit until you get used to how it feels on, and then check in with your body so you know how your testicles normally feel after the device is removed. This will help you gauge how long to leave it on in the future. Wearing the device overnight or for too many hours can result in numbness and tissue damage; the blood could coagulate and swelling can occur if it is too tight, which makes it harder to remove. If unable to remove the device, go to a hospital immediately. However, after getting used to the device, you can wear it for longer periods of time as long as you take it off at the first sign of discomfort.
Don’t put the stretcher on while in the shower. Being in the shower will actually make it more difficult to put on the stretcher. The water will wash away any lube that is applied to the testicles and scrotum.
Metal stretchers should not be exposed to water for longer than necessary. Only get the stretcher wet while cleaning it, and then make sure it is thoroughly dried. If there’s excess water exposure, the screws holding the stretcher together may start to rust.
If it hurts, stop or take it off. Pain is a great indicator that something isn’t fitting correctly; make sure to pay attention and take the device off before any damage is done. Also, it is common for men to feel pain if they have stretched too fast or gone up to a heavier weight too soon before their body is used to it and has healed from the initial stage of stretching.
If while wearing a ball stretcher the balls literally turn purple or blue, take it off. This discoloration indicates lack of blood circulation and can cause blood to clot and damage to the testicles. Readjust so there is less pinching on the scrotum or get a slightly larger size.
How do I measure to make sure the ball stretcher is the right size before buying it?
For leather:
- Take a cardboard toilet roll and fold it in half so it’s flat.
- Measure out 32mm (or 1.25 inches) away from the end and cut. Do this twice to get two pieces.
- Cut lengthwise to open up the tube so it can wrap around the scrotum.
- Take a warm shower (as you normally would to prep before putting on a real ball stretcher) to increase circulation and allow the testicles to descend.
- Overlap the two pieces and wrap them both around the scrotum (in-between the penis and the balls). Have a marker nearby.
- Begin to gently pull the balls downward, away from the body. As you do this, slide one of the toilet rolls up and the other down (so the two rolls are sliding away from each other).
- As you find the farthest point that your balls can stretch away from the body (without causing pain), the cardboard rolls will get further away from each other. At the farthest point, mark the line on the cardboard.
- Take the measurement of one roll (at 1.25 inches or 32mm) plus the measurement of where the marked line is on the other roll. This is the length for your leather stretcher.
- When looking for leather ball stretchers to buy and you are in doubt if it’s the right size, go for slightly smaller instead of too large.
For steel or rubber rings:
- Take a fabric measuring tape and wrap it snugly (but not too tightly!) around the scrotum between the penis and the balls where the ball stretcher would sit.
- Take down the measurement. This is the circumference.
- Divide the circumference by 3.14. This is the diameter and this number will be used to find the right size.
If you don’t have a fabric measuring tape:
- Take a piece of string or cord and wrap it snugly (but not too tightly) around the scrotum between the penis and the balls where the ball stretcher would sit.
- Take a marker and mark the spot where the end meets the cord/string.
- Lay the string or cord over a measuring tape and take down that measurement. That is your circumference.
- Divide the circumference by 3.14: this is your diameter. You will use this number to find the right size.
When wearing ball stretchers, consider your clothing.
Loose fitting clothing or wearing a pair of office slacks one size up gives more space for the balls to hang along the inside of one pant leg.
FAQ
How long does it take to put a ball stretcher on?
Initially, it can take up to 20 minutes to put it on the first few times. With practice, it can take as little as 3-5 minutes.
How long can I wear a ball stretcher?
When trying out a new stretching toy, give yourself a limit of 10-20 minutes to get a sense of how it makes you feel during and after the stretch.
How much lube should I use when putting a ball stretcher on?
Apply just enough to the scrotum where the stretcher will sit so that it limits any possible friction. Do not use excessive lube or else your fingers and testes will get slippery and it will be difficult to grab the pieces of the stretcher and put them on.
Is ball stretching permanent?
For those who wear them consistently for long periods of time, there can be. The men who make this a regular practice pride themselves in being able to have lower hanging balls. The skin does stretch and if stretched beyond a certain point, it may not retract again. For example, those who stretch their ears with plugs may have some elasticity to pull the skin back into place. However, if they stretch the hole large enough, it will stay stretched out.
Is ball stretching, cock rings or penis jewelry painful?
Not necessarily. Usually these items are used to enhance sexual pleasure. However, when people use them in extreme ways or intentionally painful ways, they do become implements used in CBT (cock and ball torture).
How can I learn more about what toys are right for me?
Everyone has personal preferences and the best way to find out what you like is to simply try out things in real life! If you’re not sure where to start, then pay attention to what kinds of porn you have watched, what fantasies turn you on, or what excited you when you learned about it. Also pay attention to if you enjoyed twisting, squeezing, bending, or putting pressure on your cock or balls; this may indicate an interest in CBT, even if it’s a mild form of it.
