I initially bought the medium size and it completely killed my jaw. I couldn't really use it because I was afraid my jaw would lock, and I would end up at the ER with an extremely embarassing story. In any case, I just bought the small size, and I absolutely love it! The drooling is amazing and I can wear it for an extended period without fear.

I guess my advice is that if when you get x-rays at the dentist, they feel a little uncomfortable, get the small size!
