It's a very nice toy, not too uncomfortable to wear (it's the first time I wear anything like that). It's very nice for the reasons why my girlfriend and I decided to buy it : role playing, teasing, and erotic stimulation during the day - anytime you feel the cage, or think that someone might notice, you will think about the keyholder... However one can get out of it quite easily. There are 3 locking size adjustments, but even on the tightest, which was too tight for me (I think the blood circulation was cut, I only kept it there 1 minute) I could get out without the key, just bending my flacid (~4 inches) penis out, followed by the balls. Still, I could absolutely not put it back on (on the medium adjustment, which is tight but not too tight) without opening it. So it's possible to cheat, but the keyholder will know she (or he) cannot trust the wearer. That's why I say it does its job. With the restriction that I suppose someone with small balls could put it on and off without the key. Also, it is hardly noticeable under trousers. At least the smallest size (with no additional ring). My penis pokes out of it any time I get a bit excited but it's not a problem. I think the large size would be noticeable, and probably less comfortable in trousers (Of course, one can wear a kilt). Last detail : the cage is 3.5 inches long, but while I wear it under clothes the base of the cage is one inch from the base of the penis. The diameter is almost 2 inches, but under 1.5 inches at the end
