I tried this out today, and it's such a great toy. My only worry is that the wearer does have a chance to dislodge it from the mouth if the gag has not expanded far enough. It is quite possible to add sufficient pressure with the tongue, and in conjunction with lips to remove the device.

Other than that, I think it's great.
