When I first got this gag, I was worried that it would be too big for my mouth (I'm not a very large person-only 5'5"), but it fit just fine! Occasionally, when not positioned properly, the staps bite into the corners of my mouth. This problem (or not) can usually be fixed by shifting it around in my mouth. After more than a few minutes of wearing it, your jaw might be a little bit stiff, but that fades before the after-glow. Over-all I love it! It's the only one I found that looked like something I wanted and turns every sound into a moan!
